Check if subject responds to voice. If unresponsive, perform a firm sternal rub with knuckles across the center of the chest. Check for chest rise and breathing sounds.

Harm Reduction Clinical Standards

Toxidrome Triad

Opioid overdose is characterized by three core clinical indicators:

  • Respiratory Depression: Fewer than 8-10 breaths per minute, shallow breathing, gurgling ("death rattle"), or agonal gasping.
  • Pinpoint Pupils: Miosis (pupils constricted to small points even in low light).
  • Unconsciousness: Deep unresponsiveness to verbal stimuli and vigorous sternal stimulation.

The 3-Minute Naloxone Rule

Naloxone (Narcan) temporarily displaces opioids from brain receptor sites to restore natural breathing within 2 to 3 minutes:

  • Wait 2-3 Minutes: Do not immediately pump additional doses without waiting, unless there is zero improvement after full 3 minutes.
  • High-Potency Fentanyl: May require 2 or more doses due to tight receptor binding affinity.
  • Half-Life Warning: Naloxone wears off in 30–90 minutes; synthetic opioids can remain active longer, leading to secondary respiratory arrest. EMS must transport.

Good Samaritan Protection

Legal protections exist to remove hesitation when dialing emergency dispatch:

  • Clear Dispatch Communication: Say "A person is unconscious and not breathing normally" and give the exact address first.
  • Immunity: In 48+ states and international jurisdictions, callers and responders are legally shielded from drug possession charges when seeking overdose assistance.
  • Stay on the Line: Follow dispatcher instructions and prepare to direct the paramedic team upon arrival.
